CONDITIONS OF PFA MEMBERSHIP
AS A MEMBER OF THE PFA, I AGREE:
- To pay the annual subscription fee in the time, amount and manner determined by the PFA Executive from time to time;
- To be bound by, comply with and observe the Rules of the PFA and any determination or resolutions of the PFA Executive which may be made;
- To authorise the PFA on a non-exclusive basis to use my name, image or likeness or any other indicia of identity for the purposes of the Statement of Purpose of the PFA;
- To the release of a copy of my Standard Player Contract (SPC) to the PFA, for the purpose of player advice and research.
TERMS OF DIRECT DEBIT
AS A MEMBER OF THE PFA:
- I understand that if my Direct Debit for a bank account falls on a day which is not a business day, the PFA will direct my bank/financial institution to debit my account on the next business day;
- I agree to pay any fees charged by my bank/financial institution associated with insufficient funds being unavailable at the time of Direct Debit
- I understand that I can contact the PFA on 1300 650 497 if I am unaware of the date of my next Direct Debit;
- I understand that I am responsible to inform the PFA of any changes to the details I have provided on this form;
- I understand that this arrangement for payment will continue until I formally resign my membership of the PFA in accordance with the Rules of the PFA (available at www.pfa.net.au) and/or request for the Direct Debit to stop by contacting the PFA or my bank/financial institution;
- I understand that if I wish to stop this Direct Debit I may do so by contacting the PFA by email to info@pfa.net.au; and
- I understand that such a request to stop the Direct Debit will not be a request to cancel any ongoing contracts or arrangements with the PFA and I will continue to be liable under those contracts or arrangements
